Data Processing Policy for Personal Data As “Assoc. Prof. Dr. PELİN ÖZTÜRK,” we take utmost care in the processing and protection of your personal data. In accordance with the Personal Data Protection Law, Basic Law on Health
Services, Regulation on Personal Health Data, Patient Rights Regulation, and relevant legislation, necessary technical and administrative measures are taken as the data controller to prevent the unlawful processing of personal data, prevent unauthorized access to personal data, and ensure the preservation of personal data.
In accordance with Article 10 of the Personal Data Protection Law, we inform you through the policies created for our patients and their relatives and this disclosure text.

This disclosure text has been prepared by “Assoc. Prof. Dr. PELİN ÖZTÜRK” as the data controller within the scope of the 10th article of the Personal Data Protection Law (“Law”), the 5/8th article of the Regulation on Personal Health Data, and the Communiqué on Principles and Procedures to be Followed in Fulfilling the Obligation to Inform.

Data Controller:

Prof. Dr. PELİN ÖZTÜRK” hereinafter referred to as the “CLINIC.” The personal data defined as “Data Controller” in Article 3 of the Personal Data Protection Law are processed.

Purpose of Processing Personal Data:

Your personal data collected in accordance with the Personal Data Protection Law, Basic Law on Health Services, Regulation on Personal Health Data, Patient Rights Regulation, and relevant legislation are processed by “CLINIC” within the framework of the purposes listed below, fully or partially, automatically or non-automatically, recorded, stored, modified, reorganized, and processed as part of any data recording system:

  • Protecting public health, conducting preventive medicine, carrying out medical diagnosis, treatment, and care services, ensuring the planning and management of health services financing,
  • Ensuring the preservation of data required to be kept by legislation within the scope of the health services provided,
  • Confirming your identity, confirming your legal contact with contracted institutions in the context of the provided health services, billing, and financial reconciliation,
  • Conducting medical diagnosis, examination, treatment, and care services to protect public health and for preventive medicine,
  • Providing information to public prosecutors, courts, and relevant public officials in matters related to public safety and legal disputes, upon request and in accordance with the legislation,
  • Appointment scheduling, creation of appointments, provision of necessary information, ensuring patient and patient-relative satisfaction, monitoring request and complaint processes, conducting necessary examinations and evaluations for the provided health services,
  • Providing health and counseling services, including preventive health services,
  • Planning and managing the internal workings of the clinic, developing services, conducting analysis, risk management, and evaluating quality processes,
  • Issuing freelance invoices for the services provided;
  • Improving clinic services, continuing development activities, managing financial, accounting, administrative, legal, technical business processes, risk management, and quality improvement processes,
  • Meeting the burden of proof in case of possible legal disputes with patients,
  • Maintaining processes necessary for filling out physical forms, ensuring the transaction security of relevant individuals,
  • Providing necessary information to regulatory and supervisory public institutions, private legal entities,
  • Ensuring the invoicing and payment processes related to the services provided;
  • Your personal data obtained by the Doç. Dr. Pelin Öztürk clinic will be used primarily to provide you with effective, secure, and quality healthcare services. Among the general purposes of using your personal data are identity verification, establishment of doctor- patient communication, protection of public health, conduct of preventive medicine, planning and efficient execution of medical diagnosis, treatment, and care services, planning and management of health service financing, planning and management of the internal workings of the clinic, daily operation planning, and management, procurement of drugs and materials. We will use your data to inform you about appointments if you make an appointment at our clinic.


In line with the above-mentioned purposes, your personal data are processed within the scope of clinic activities, and the following personal data are processed:

  • Your identity information (T.C. ID number, your name and surname, your ID card, or other ID information on the identity sharing system),
  • Your contact information (phone numbers, contact address, email address),
  • Financial information (billing and payment information, bank account number, IBAN number, credit card information),
  • Visual and audio recordings (photograph information on official documents and identity documents),
  • Health information (personal data related to patients processed within the scope of activities such as examination, all kinds of laboratory, imaging, and test results, patient diagnosis, diagnosis, treatment, prescription, medication information, doctor analysis and comments, diagnosis and prescription information, health reports, nutrition, diet information, and all health information obtained within the scope of health services, health board reports, personal data related to diagnosis and treatment processes).
  • To Whom and for What Purpose the Processed Personal Data Can Be Transferred:


Your personal data processed by our clinic are transferred to the real and legal persons mentioned below in accordance with the relevant legislation and the 8th and 9th articles of the Personal Data Protection Law for the purposes listed below:

  • In cooperation for the realization of medical diagnosis, examination, treatment, and referral processes, university hospitals, public hospitals, private hospitals, medical faculties, other healthcare institutions, banks, financial institutions, public and private legal entities for the realization of financial transactions of the relevant individuals,
  • In matters related to public safety and legal disputes, upon request and limited to the purpose of the request, to public prosecutors, courts, arbitrators, execution offices, and relevant legal institutions and organizations,
  • For the transfer of personal data abroad, in cases required by legal regulation for special categories of personal data, without the need for explicit consent and limited to the purpose of the transfer for the protection of public health, preventive medicine, the conduct of medical diagnosis, treatment, and care services, and the planning and management of health services and financing, your personal data are transferred in accordance with the rules specified in the 5th and 6th articles of the Personal Data Protection Law and the 5/1-h. provision of the Communiqué on Principles and  Procedures to be Followed in Fulfilling the Obligation to Inform. This transfer is limited to clinic activities and may include your personal information such as identity, contact, transaction security, service area/customer transaction information, financial information, visual and audio data, health information, genetic data, protocol number, and patient number, through applications used, software programs, websites, mobile applications, social media accounts.
  • Duration: Your personal data will be processed in accordance with the mandatory periods specified in the legislation, including but not limited to the Personal Data Protection Law, as long as the legitimate purposes mentioned above continue. After the specified period has passed, your personal data will be deleted, destroyed, or anonymized by the clinic in accordance with the methods specified in the Personal Data Protection Law and related regulations, either by the clinic or upon your request. You can always withdraw your consent for the processing of personal data other than data required by law.
  • Data Transfer Abroad: In accordance with the principles set out in the Personal Data Protection Law, your personal data may be transferred abroad in line with the purposes stated in the 3rd article and within the framework of the conditions stipulated in the 9th article of the Law.
  • Rights of the Data Subject: In accordance with Article 11 of the Personal Data Protection Law, you have the right to request:

  • Requesting information about processed personal data,
  • Learning the purpose of processing personal data and whether they are used appropriately for their purpose,
  • Knowing the third parties to whom personal data is transferred domestically or abroad,
  • Requesting correction of personal data in case of incomplete or incorrect processing,
  • Request the deletion or destruction of personal data within the framework of the conditions stipulated in Article 7 of the Personal Data Protection Law,
  • Requesting the notification of the transactions made pursuant to Articles 5 and 6 to third parties to whom personal data has been transferred,
  • To object to the occurrence of a result against the person himself by analyzing the processed data exclusively through automated systems,
  • In case of damage due to unlawful processing of personal data, request the compensation of the damage. In order to exercise your rights mentioned above, you can send your request including your necessary identification information and explanations about the right you want to use through the methods specified in the Application Form to be prepared by the Personal Data Protection Board,

Legal reasons and methods for personal data collection are as follows:
  • Your personal data collected through all kinds of verbal, written, or electronic platforms to benefit from the health services provided by the clinic within the scope of the personal data processing conditions specified in Articles 5 and 6 of the Personal Data Protection Law and within the scope of the Health Services Basic Law and relevant legislation.
  • Your personal data shared verbally, in writing, or electronically during the registration process, face-to-face or remotely, with our clinic for health services and for the provision of health services,
  • Personal data collected through all kinds of verbal, written, or electronic platforms, such as examination, diagnosis, treatment, and care services to be provided by our clinic,
  • Personal data collected to ensure the planning and management of health services financing,
  • Personal data collected to confirm your identity, to confirm your legal contact with contracted institutions within the context of the health services provided, and to provide appointment scheduling, appointment creation, and appointment information, and patient-relative satisfaction,
  • Personal data collected during the creation of freelance invoices for the services provided by our clinic,
  • Personal data collected to maintain processes necessary for the filling out of physical forms, transaction security of the relevant individuals,
  • Personal data collected to provide necessary information to regulatory and supervisory public institutions and private legal entities.

Your personal data are collected for specific, explicit, and legitimate purposes by processing them in connection with the purposes for which they are collected, limited and measured, and in accordance with the basic principles of personal data processing specified in Article 4 of the
Personal Data Protection Law.
